Start with the header: date, location, and home/away team names. As goals happen, log the period, time, scoring player's number, and any assists in the scoring summary table. Track penalties separately with the period, time, player number, infraction, and minutes served. Fill in goalie stats and final scores at the end of the game.

At minimum, track goals, assists, and penalties by period. Most scorekeepers also log goalie saves and goals against per period, plus the final score. This sheet includes a section for each so you have a complete record of the game.
